When is a second opinion recommended in neurology?
- With a new diagnosis of a chronic condition, such as epilepsy, multiple sclerosis or Parkinson’s disease.
- When seizures continue despite trying more than one suitable medicine; the case may need assessment at a specialist epilepsy centre.
- When a brain tumour or a brain aneurysm is found, or when surgery on the brain or spinal cord is recommended.
- With recurrent headaches or unexplained neurological symptoms despite tests.
Documents you need
- MRI or CT images of the brain or spine in their original format, with reports.
- EEG reports, and recordings if possible.
- Nerve conduction and EMG studies, if done.
- Laboratory results, including spinal fluid analysis if done.
- A list of the medicines you have tried, their doses, how long you took each and why it was stopped.
- A diary of symptoms or seizures with dates, and a video of a seizure if possible, which is very helpful for diagnosis.
Questions to ask the neurologist
- How certain is the diagnosis, and are there other possible diagnoses?
- What treatment options are available, and what are their long-term side effects?
- Is surgery or an interventional procedure suitable for me, and what are the expected risks?
- What should I watch for, and when do I need urgent review?
Stroke warning signs: do not wait
If there is sudden facial drooping, weakness in an arm or leg, difficulty speaking, or the worst sudden headache of your life, call 997 immediately. Stroke treatment depends on time, and this is not the place for a second opinion.
Frequently asked questions
Can MRI images be reviewed remotely?
Yes. Original images can usually be reviewed remotely if they are sent in full format on a disc or link, not as screenshots.
When is epilepsy considered drug-resistant?
Epilepsy is usually described as drug-resistant when seizures continue after trying at least two suitable medicines at adequate doses. These cases benefit from assessment at a specialist centre.
Does every brain aneurysm need treatment?
No. The decision depends on the size and location of the aneurysm and other factors. Some cases only need regular monitoring, which is one of the most important things a second opinion discusses.
