When is a second opinion recommended in oncology?
Many patients seek a second opinion after a cancer diagnosis, and it is an accepted and common practice in oncology. It is especially useful in these situations:
- A new cancer diagnosis before treatment starts, especially if the plan involves major surgery or intensive chemotherapy or radiotherapy.
- A rare tumour type, or a diagnosis that is still uncertain between more than one possibility.
- The disease has come back after treatment, or is not responding to the current treatment.
- There are several similar treatment options, or you want to know whether a clinical trial may suit you.
Why pathology and imaging review matters
In oncology, a second opinion is more than reading the reports. An accurate diagnosis depends on examining the tissue sample (pathology) and staging the disease with imaging. The second consultant may therefore ask to review the tissue slides and the original scans, not only the written report. This review can sometimes change the tumour type or stage, and with it the treatment plan.
Documents you need
- The pathology report, with immunohistochemistry and any molecular or genetic test results.
- Tissue slides or blocks if requested; you can ask the hospital laboratory for them.
- The original scans on a disc or link (such as CT, MRI and PET) with their reports.
- The staging report, and the summary of the multidisciplinary tumour board if your case was discussed there.
- Laboratory results, including tumour markers if they were requested.
- Details of any previous treatment: medicines, doses, number of sessions and response.
Questions to ask the oncologist
- What exactly is the tumour type and stage, and do you agree with the current diagnosis?
- Are there molecular or genetic tests that could change the treatment options?
- What is the goal of the proposed treatment: cure or disease control?
- What are the alternatives, and what side effects should I expect from each, including effects on fertility and quality of life?
- Has my case been discussed by a multidisciplinary tumour board, and is there a suitable clinical trial for me?
Does a second opinion delay treatment?
For many cancers, a short wait to get a second opinion does not affect the outcome, and it can be arranged in parallel with the required tests. Some fast-growing cancers, however, such as certain blood cancers, need treatment to start without delay. Ask your treating doctor clearly how long it is safe to wait in your case.
Frequently asked questions
Will my doctor be upset if I ask for a second opinion?
Asking for a second opinion is common practice in oncology, and most doctors welcome it and help prepare the reports. You can simply tell them you want to feel confident about the decision.
Are the written reports enough?
Not always. In oncology, many consultants prefer to review the tissue slides and original scans, because a report is a summary that may not show every detail.
What if the two opinions differ?
Discuss the reasons for the difference with both doctors, and ask for your case to be reviewed by a multidisciplinary tumour board to reach a documented recommendation.
