When is a second opinion recommended in orthopedics and spine?
- When surgery is recommended for a herniated disc, spinal fusion or spinal canal decompression.
- Before a knee or hip replacement, especially if physiotherapy and other options have not been tried enough.
- For sports injuries, such as an ACL or meniscus tear, when choosing between surgery and rehabilitation.
- When pain continues after a previous operation.
It is important to know that an MRI may show disc changes in many people without them being the cause of pain. A consultant therefore links the images with your symptoms and clinical examination before recommending surgery.
Scans and reports you need
- The original MRI images on a disc or link, with the report.
- X-rays; for joints and the spine, ideally taken while standing.
- CT scans, if available.
- Nerve conduction and EMG studies, if done.
- A summary of previous physiotherapy: how long and with what result.
- Reports of any previous operation, and a list of medicines and injections you have had.
Questions to ask before surgery
- What is likely to happen if I continue conservative treatment, such as physiotherapy, medicines and injections, for longer?
- How much improvement can I expect after surgery in a case like mine?
- What are the risks, and how likely is another operation later?
- How long is rehabilitation, and when can I return to work and driving?
Signs that cannot wait
If back pain comes with loss of bladder or bowel control, numbness around the buttocks and groin, or sudden worsening weakness in the legs, go to the emergency department immediately or call 997. These signs need urgent assessment, not a planned second opinion.
Frequently asked questions
Does every herniated disc need surgery?
No. Many disc problems improve over weeks to months with conservative treatment. Surgery is usually considered when significant symptoms persist or there is nerve weakness.
Do I need a new MRI for a second opinion?
Not always. If your MRI is recent and clear, sending the original images is usually enough. A new scan may be requested if your symptoms have changed.
Is physiotherapy an alternative to joint replacement?
In early stages, physiotherapy and activity changes may delay the need for surgery. In advanced arthritis with persistent pain, joint replacement may be the better option.
